The LCI ILS 8 approach plate says ADF or DME is required. But all the
necessary fixes (including the MAHWP) can be identified by the VORs, ILS,
and OM. So why the requirement for ADF or DME?

I will guess that the fix displacement error occurring by using the OM to
identify BLNAP is excessive, requiring the use of either ADF or DME at that
location. The allowable fix displacement error varies depending on the
length of the subsequent segment.
